Why Our Work Matters

Guided by our mission to foster a healthy and equitable Pajaro Valley, we support individuals and families across the tri-county region of Santa Cruz, Monterey, and San Benito Counties through advocacy, collaboration, and wellness.

Many individuals and families face barriers to care, nourishment, and wellness that go beyond access alone. Trust, cultural understanding, and safe spaces are essential to meaningful support.

Our work responds to these realities by creating welcoming environments where people feel seen, heard, and supported. We focus on strengths, relationships, and long-term well-being rather than short-term fixes

Food Programs

Our Food Programs enhance access to fresh food, bolster local food systems, and promote community well-being.

We distributed $17,000 worth of produce and grocery items to support the well-being of our patients.

Served 159 patients, reaching 562 people total, when including household members.

Wellness Programs

Our Wellness Programs are open to everyone without a referral and aim to improve quality of life.

Offered screenings at 4 locations across Santa Cruz & Monterey County.

Provided 291 community members with free health screenings and personalized nutrition and lifestyle guidance from a Nurse Practitioner and Registered Dietitian

VeggieRx

Veggie Rx increases access to fresh produce for participants with specific health needs while supporting local farmers and the regional food system.

  • Diabetes management support focused on long-term health

  • Services supported all ages, with 35% of visits for youth and 65% for adults.
