Care & Nutrition

Care Programs

Our Care Programs provide medical, behavioral, and social support through family-centered services that improve health and well-being. Through coordinated care, we help individuals and families achieve better health outcomes.

Nutrition Services

(formerly known as the Diabetes Health Center)

Nutrition Services provides personalized guidance from Registered Dietitians to help individuals manage chronic conditions, improve nutrition, and build lifelong healthy habits. Through one-on-one support, participants gain practical tools to make informed food choices and support their overall well-being.

Founded in 1998 as the Diabetes Health Center, this program has long been part of CHT’s commitment to equitable health care. Today, it continues to grow to meet a wider range of nutrition and wellness needs across the community.

VeggieRx

VeggieRx uses food as medicine to improve community health, strengthen local food systems, and support small farmers. Through weekly deliveries of fresh, locally sourced produce, participants build healthy habits, increase fruit and vegetable consumption, and manage chronic health conditions such as diabetes, heart disease, and obesity.

In addition to produce deliveries, participants receive personalized guidance from Registered Dietitians, including nutrition tips, simple recipes, and support for preparing healthy meals at home. This holistic approach helps reduce hospital readmissions, improve health outcomes, and empower individuals to take control of their nutrition.

This program is supported in part by Central California Alliance for Health (CCAH).
