#LIVE Pajaro Valley Initiative
Convened by the Community Health Trust in 2018, #LivePV initiative is a cross-sector response to the social inequities that prevent residents of the Pajaro Valley, a place of relative abundance, from achieving fullness of health and quality of life.
Goals
- Advance equity through partnership; work together across sectors to address the complex issues that deter individual and community health.
- Maintain and strengthen existing collaborative initiatives and build from what is already working well.
- Inspire a cross-sector of community leaders and residents to identify innovative solutions to long-standing issues and together redesign the future of health and wellness in the Pajaro Valley.
Regional Diabetes Collaborative (RDC)
In 2001, the Community Health Trust convened a Regional Diabetes Collaborative (RDC) comprised of more than 40 public and private member agencies, as well as medical providers and consumer representatives to support, promote, and coordinate efforts to prevent and manage diabetes in Monterey, San Benito, and Santa Cruz Counties.
Goals
- Partnerships with private, public, governmental, and community-based organizations to prevent diabetes.
- Increased public awareness and diabetes education.
- Promotion of best practices in clinical management to improve treatment effectiveness for diabetes.
- Improved availability, accessibility, and affordability of treatment services, medicines, and equipment for diabetes patients.
- Development and implementation of a policy advocacy plan designed to address diabetes.

Central Coast Oral Health Collaborative (CCOHC)
The Central Coast Oral Health Collaborative (CCOHC) was convened by the Pajaro Valley Community Health Trust in July 2007 in an effort to improve the oral health status of the Central Coast’s low-income and underserved populations by advocating for increased prevention, access, and awareness.
Goals
- Increase access and decrease barriers to oral health services;
- Decrease the burden of oral disease through promotion of oral health and primary prevention; and
- Advocate for policy reforms at the state and local levels that improve the oral health of underserved populations in our community.
In 2016, Health Trust grantee-partner, Dientes Community Dental Care (Dientes) commissioned the first-ever Santa Cruz County Oral Health Needs Assessment. Completed in 2017, the assessment shaped the County’s first Oral Health Access Strategic Plan.
To unite efforts, the CCOHC has since adopted the following goals central in the County’s Oral Health Strategic Plan and joined the efforts of the Oral Health Access Santa Cruz County Steering Committee:
- Launch a First Tooth First Birthday education campaign to reach new parents and their healthcare providers.
- Promote Kindergarten oral health screenings.
- Expand treatment, prevention, and clinical capacity.
