VeggieRx is designed to promote healthy eating, increase vegetable consumption, support small farmers, and strengthen the local food system.
Designed for Central California Alliance for Health (CCAH) members with specific health conditions, this program provides participants with a 12-week supply of medically tailored produce boxes or grocery items delivered directly to their homes.
To ensure maximum impact, participants receive two 30-minute sessions with a Registered Dietitian to develop and implement personalized nutrition strategies.
To qualify, CCAH member must meet one of the following requirements:
- Diabetes Type I or Type II
- Congestive Heart Failure
- Chronic Obstructive Pulmonary Disease (COPD)
- Cancer
- Obesity
- Gestational Diabetes
- High-Risk Pregnancy
- HIV
- Heart Disease (CAD, HTN, etc)
- Stroke or History of Stroke
- Mental Health Disorder
- Chronic Kidney Disease
