OPENING FALL 2026!

Centro de Bienestar—A Wellness Center for the Pajaro Valley!

We’re excited to announce that our Diabetes Health Center (DHC) is transforming into a wellness center—Centro de Bienestar—in Downtown Watsonville at 112 E. Beach St.!

Set to open in the fall of 2026, this expanded space will further CHT’s mission to provide equitable access to resources that promote health, prevent illness, and improve quality of life.

For over 25 years, the Community Health Trust has been a trusted leader in reducing health disparities and advancing health equity. Our work began in 1998 with the creation of the Diabetes Health Center—a direct response to the overwhelming number of patients arriving at Watsonville Community Hospital with uncontrolled diabetes.

Today, our strategic plan builds on this legacy. We are expanding beyond diabetes care to deliver comprehensive, integrated wellness services, transforming the Diabetes Health Center into Centro de Bienestar, a center built to meet the evolving needs of Pajaro Valley families. 

A Family-Focused Health Hub

Centro de Bienestar will be a place where families can come together to learn, bond, and embrace health as a shared value. There will be opportunities for families to build lasting habits that benefit everyone, now and for generations to come.

Expanding Beyond Diabetes Care

Centro de Bienestar will offer diabetes management, chronic disease care, nutrition education, healthy living programs, fitness classes, and more! We aim to provide access to high-quality, holistic care, ensuring everyone in the Pajaro Valley can thrive. 

Comprehensive, Whole-Person Care

With a multidisciplinary team including a Wellness Nurse Practitioner, Registered Dietitians, Diabetes Educators, and Community Health Workers, we provide personalized care and resources to support your health journey. Our team works together to help you navigate healthcare, build healthy habits, and achieve lasting well-being. 

Bringing Food, Wellness, and Care Together

Our Food Programs increase access to fresh, healthy food, strengthen local food systems, and support community well-being, including Community Gardens, El Mercado, Mini Mercado, and the Food, Farming & Health Policy Council.

Our Wellness Programs are open to all—no referral needed—and promote access to services that enhance quality of life, such as Community Health Services, Health & Edible Education, and Active Wellness.

Our Care Programs provide medical, behavioral, and social support through family-centered services designed to improve health outcomes, including Community Health Workers, Enhanced Care Management, Nutrition Services, and VeggieRx.

Centro de Bienestar brings together programs that have long supported families in the Pajaro Valley into one shared, accessible space. These services have existed across the community for many years, shaped by local needs and long-standing partnerships.

When programs are spread across multiple locations, consistent participation can be more difficult for families balancing health needs, work, and daily life. Centro de Bienestar was developed to reduce that complexity by creating a place where wellness resources are easier to access and navigate.

By centralizing health education, chronic disease support, nutrition guidance, and wellness activities, the center supports ongoing engagement rather than one-time visits. Families can build routines and connect with resources in ways that fit their schedules and priorities.

As a community hub for the Pajaro Valley, Centro de Bienestar organizes care and wellness resources in one place—guided by the needs and experiences of the community it serves.

Part of a Larger Healthy Living & Wellness Campus

Centro de Bienestar will be located within a broader Healthy Living & Wellness Campus, alongside partners such as allcove™ Watsonville and Somos Watsonville.

Opening in 2026, allcove™ Watsonville will be part of a statewide network of integrated, holistic mental health centers for young people ages 12-25. Based on a model developed by Stanford University’s Center for Youth Mental Health and Wellbeing, allcove centers are built with, by, and for youth. CHT is proudly serving as the lead agency in bringing this initiative to the community.

Centro de Bienestar represents a long-term investment in community infrastructure. Support sustains a shared space where wellness, food, and care programs work together, remain accessible, and continue responding to community needs across the Pajaro Valley. Through partnerships and contributions, resources strengthen program coordination, activate the space, and ensure families can continue to rely on these services.
