The largest program of the Community Health Trust, the Diabetes Health Center (DHC), provides education and support for those living with diabetes or prediabetes and other health conditions. We strive to empower clients and provide resources and tools to help build skills to make informed decisions to improve quality of life and reduce the risk of complications for many medical conditions and health problems.

Diabetes Self-Management Education and Support

Learn how to manage your diabetes and related conditions. DSMES is tailored to you to meet and support your individual needs, goals and life experiences to improve your quality of life and reduce the risk of complications. DSMES includes our group classes and individual sessions.

Diabetes Health Factors and COVID-19

Gestational Diabetes Management Education

We’ll provide support and education through your pregnancy to keep you and your baby healthy and reduce the risk of developing Type 2 diabetes later in life. This comprehensive program is available in English and Spanish for pregnant women with, or at risk, of developing diabetes.

Program includes:

  • An assessment with an RN
  • Medical nutrition assessment and education
  • Insulin injection instruction, if needed
  • Follow-up visits every 1-2 weeks, based on patient need
  • Progress reports for the referring physician following each visit

Group Classes

We’ll provide basic DSMES education and support in a group setting. Group classes are facilitated by our Certified Diabetes Care and Education Specialists (CDCES). Learn to live with diabetes and get ideas for success.

  • Monitor your glucose and interpret testing results
  • Manage your medications
  • Understand the connection between blood sugar and food
  • Strategize meal planning, healthy eating, and physical activity
  • Keep healthy and prevent complications

You can attend two 90-minute classes in person at the Diabetes Health Center (DHC), or virtually* in addition to your individual assessment with one of our educators. Call to register!

Healthy Cooking Demos

Want to learn how to make healthy, delicious food? Our nutrition experts offer step by step demonstrations of how to cook healthy, budget-friendly recipes with easy to get ingredients. Plus in-person participants receive a FREE bag of produce! Classes take place in the Kathleen King Community Room at 85 Nielson St., Watsonville. Social distancing guidelines will be followed and participants will be required to wear a mask.

Medical Nutrition Therapy

Certain medical conditions can be treated with changes in diet – this is where Medical Nutrition Therapy (MNT) comes in. Our dietitians will work one on one with you to address your medical condition and help to lower the risk of new complications.

The MNT Program offered through the Diabetes Health Center is designed for any individual living with acute and chronic conditions that can be improved or managed through simple lifestyle changes and healthier habits. This program is available for women, men, teens, children, and older adults.

NOTE: A physician referral including a diagnosis and any pertinent labs is needed for MNT services. Some participants may have a co-pay.


Telehealth enables you to have a video visit with your dietitian or diabetes educator without leaving your home. You can use your smartphone, tablet or laptop to
attend your appointment virtually.
